How to Access Our NDIS Supports

At Villa Healthcare Services, we make it simple and supportive for participants to access the services they need. Our process is designed to be clear, personalised, and focused on your goals every step of the way.

Referral / Intake Form

Getting started is easy. Participants, families, or support coordinators can complete our referral or intake form online or contact our team directly.

We collect key information about your needs, goals, and NDIS plan to ensure we provide the right support from the beginning.

Introductory Meeting

Once we receive your enquiry, we organise an initial meeting (in person or online).

This is an opportunity to:

  • Get to know you

  • Understand your goals and preferences

  • Discuss how we can support your NDIS journey

Participant Onboarding

After you decide to proceed, we guide you through a smooth onboarding process.

This includes:

  • Service agreements

  • Risk assessments (if required)

  • Care planning tailored to your needs

Support Matching

We carefully match you with the right support workers based on your preferences, needs, and personality.

Our goal is to ensure you feel comfortable, supported, and empowered with your team.

Review Supports

We regularly check in to make sure your supports are working well and meeting your goals.

Adjustments can be made at any time to ensure the best outcomes.

Continuous Monitoring & Improvement

We are committed to delivering high-quality, person-centred care.

Through ongoing monitoring, feedback, and quality reviews, we continuously improve our services to better support you.
